Tips for New & Returning Patients
Please read the following before scheduling your appointment:

- Taking ownership of your health is an important first step. At PROVOKE Health, we’re committed to motivating, guiding, and supporting our patients on their journey to regaining their confidence, resiliency, and health. - We offer multiple types of appointments (our appointments tend to be more in-depth than you may be used to) :
- Telehealth / Video and Phone Appointments: Our telehealth appointments use the HIPAA-compliant version of Zoom. If videoconferencing isn’t your thing, we can meet by phone. All of our telehealth and phone appointments are booked using the scheduling tool available below.
- In-Person Clinic Appointments: In-person clinic appointments are held in either our Lutz, Florida, location or our South Tampa, Florida, location. Lutz appointments (which take place at 21754 State Road 54, Suite 102; Lutz, Florida 33549) are booked using the scheduling tool below. - Not in Tampa? Using the HIPAA-compliant version of Zoom (as well as the phone), we consult with patients from across the United States and abroad. If a virtual consultation interests you, here’s some additional information:
- Our healthcare team members have been providing virtual consultations since 2017. The feedback has been overwhelmingly positive. Patients are truly excited about the many conveniences — including not having to travel, sit in traffic, miss work, find childcare or bring children to the office.
- New to Telemedicine? Patients often tell us they feel more comfortable in their home environment, where there’s a profound reduction in their anxiety. That’s because some of the patients we see are already feeling overwhelmed and judged about their health. That holds especially true for those who have been to see multiple healthcare providers, specialists, and even hospital emergency rooms or urgent care clinics. They are tired of waiting around, traveling, and not getting the type of care they need and deserve. Telemedicine changes all of that. At PROVOKE Health, we recognize the shortcomings of conventional healthcare, which is why we offer functional medicine and integrative healthcare that empowers and motivates you to regain your confidence, resiliency, health, and overall well-being.
- We Have a Confession to Make: We love telemedicine consultations. After all, a happy relaxed doctor or nurse practitioner is refreshing for many patients, especially those who are used to being rushed during visits. A healthcare team member who is rapidly moving from room to room just putting out fires, is not always in the best state of mind for patients.
- We have a 48-hour cancellation policy, which means there are no refunds if an appointment isn’t rescheduled 48 hours in advance of the time for which it was booked.
- New to Functional Medicine and Integrative Healthcare? We take a Functional Medicine approach to healthcare. That means that when you choose to book an appointment with us, you are reserving our time to help you get at the root cause of whatever it is that is ailing you. In addition, you are seeing a healthcare professional who is experienced in creating and implementing a highly customized treatment plan to make you healthy and whole again. - New Patients: Prior to your first visit, please complete your new patient paperwork, which will be emailed to you from info@ProvokeHealth.com. Your responses will provide our healthcare team with valuable information before your first appointment — information that provides us with a deep appreciation for your concerns and health history.
- During your first visit — regardless of if we meet online, by phone, or at the PROVOKE Health clinic in Lutz or South Tampa — please be prepared to spend at least 60 minutes with us. At the end of our first appointment, don’t be surprised if I write multiple orders for blood tests, suggest dietary and/or intermediary lifestyle changes, or ask you to make other changes that we believe will relieve symptoms or help us arrive at an initial diagnosis.
- Lab tests are often ordered to check hormone levels, identify any vitamin or mineral deficiencies, evaluate thyroid function, check cortisol levels, examine lipids (fatty acids), and identify markers for inflammation. These lab tests are likely to be the most comprehensive you have ever had, and there is a good reason for that: As your healthcare team, we are not interested in merely treating symptoms. Instead, we identify and treat underlying issues — whatever is preventing your body from functioning in a fully healthy manner.
